How to Add a Last Name Question
To add a Last Name field into your survey:
Go to the Survey Hub and select the survey you want to edit.
Navigate to the Build section.
From the Question Types Panel, find and select Last Name under the Contact Information category.
Drag and drop the Last Name field into your survey.
Click on the field to edit the label text, for example: “Please enter your last name”.
Customization options
Question Label: Customize wording as needed
Make Required: Ensure submission only after the last name is entered
Placeholder Text: Add hint text inside the field (e.g., “Enter last name”)
