How to Add a Last Name Question

To add a Last Name field into your survey:

  • Go to the Survey Hub and select the survey you want to edit.

  • Navigate to the Build section.

  • From the Question Types Panel, find and select Last Name under the Contact Information category.

  • Drag and drop the Last Name field into your survey.

  • Click on the field to edit the label text, for example: “Please enter your last name”.


Customization options

  • Question Label: Customize wording as needed

  • Make Required: Ensure submission only after the last name is entered

  • Placeholder Text: Add hint text inside the field (e.g., “Enter last name”)

  • Validation: Restricts responses to valid characters for clean data


Things to keep in mind

  • Data collected here syncs automatically into Contacts

  • Use alongside First Name or Full Name for complete records
