How to Add a Full Name Question
To add a Full Name field into your survey:
Go to the Survey Hub and open the survey you want to edit.
Navigate to the Build section.
From the Question Types Panel, find and select Full Name under the Contact Information category.
Drag and drop the Full Name field into your survey.
Click on the field to edit the question label, for example: “Please enter your full name”.
